FIELD TRIP FORMS

Fillable pdf forms must be completed by staff member and distributed to parents. If using computer to complete staff portion:
  1. Fill out staff portion of fillable form on computer.
  2. Save to computer desktop using a unique file name.
  3. Distribute via email or print to parents for signatures.

You may also print out the forms and complete the staff portion by hand.
